Support Worker
Location: Oxfordshire

Reports to: Head of Complex Care
Working with: Clinical Lead for Complex Care, RMNs, Senior Management Team, Operations Manager, Business Development Manager, Healthcare providers

What you will do:
You will provide outstanding care and support to individuals with mental health conditions under CQC regulated activities in England. This will involve supporting these individuals with daily activities, promoting independence and supporting their overall well-being.
What are you responsible for:
- Supporting patients with everyday tasks such as personal hygiene, meal preparation and household chores
- Prompting and supporting patients to take their medications as prescribed and monitor for any side effects or issues
- Monitoring patients’ physical and mental health, noting any changes in behaviour or symptoms
- Provide a listening ear, offer encouragement and help patients manager their emotions
- Encourage and facilitate participation in social activities, helping patients to build and maintain relationships
- Teach and support patients in developing skills for independent living
- Work with patients to follow individualised care plans developed by healthcare professionals, ensuring that the care plan goals are being met
- Maintain accurate and up-to-date records of patient interactions, progress and any incidents or changes in condition
- Communicate regularly with healthcare professionals, providing updates on patients progress and any concerns or change that need attention
- Respond to crisis, providing immediate support and intervention to ensure the safety and well-being of patients
- Help develop and implement safety plans for patients at risk of self-harm or experiencing severe mental health episodes
- Advocate for patients’ rights and needs within the healthcare system and in the community
- Empower patients to make decisions about their own care and to express their needs and preferences
- Work closely with a team of healthcare professionals, including nurses, social works and therapists to provide co-ordinated care
- Engage with patients’ families or caregivers, providing support and education to help them understand and assist in the care process

- Minimum of 2 years’ experience within a similar role
- Health & Social Care
- NVQ or willingness to work towards NVQ
