We are seeking Support Workers to join our team and provide exceptional care and support to young people and adults with complex needs in the community. As a Support Worker you will play a crucial role in promoting independence, supporting with care plan interventions and enhancing the quality of life for our clients
What are you responsible for:
• Provide one-on-one support to individuals with complex needs.
• Assist clients with daily living activities, personal care, and medication management.
• Foster a safe and supportive environment, promoting physical and emotional well-being.
• Collaborate with a multidisciplinary team to ensure care plans are being adhered to.
• Support clients in building life skills and participating in daily social/community activities.
• Document client progress, communicate observations to the Clinical Lead or Registered Manager.
• Provide emotional support, managing challenging behaviours and ensure clients’ voices are heard.
Required skills:
• Compassionate and empathetic with a strong desire to support others.
• Previous experience in a support worker role is advantageous, minimum 3 years
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• A commitment to promoting independence and enhancing the lives of individuals with complex needs.
• Flexibility to work a variety of shifts, including evenings, weekends, and holidays.
• NVQ Level 3 in Health and social care
To be considered for this position, you will demonstrate exposure to the following areas of expertise: mental health, complex health needs, physical health needs, hospital discharge to community rehabilitation, autism and learning disabilities, life-limiting conditions, working with children and young people / adults aged 18 – 65 and 65 and above.
