Are you a caring and dedicated individual with a passion for making a positive impact on the lives of others? We are seeking Support Workers to join our team and provide exceptional care and support to young people and adults with complex needs in the community. As a Support Worker you will play a crucial role in promoting independence, supporting with care plan interventions and enhancing the quality of life for our clients

What are you responsible for:
•    Provide one-on-one support to individuals with complex needs.
•    Assist clients with daily living activities, personal care, and medication management.
•    Foster a safe and supportive environment, promoting physical and emotional well-being.
•    Collaborate with a multidisciplinary team to ensure care plans are being adhered to.
•    Support clients in building life skills and participating in daily social/community activities.
•    Document client progress, communicate observations to the Clinical Lead or Registered Manager.
•    Provide emotional support, managing challenging behaviours and ensure clients’ voices are heard.

Required skills:
•    Compassionate and empathetic with a strong desire to support others.
•    Previous experience in a support worker role is advantageous, minimum 3 years
•    Excellent communication and interpersonal skills.
•    Ability to work independently and as part of a team.
•    A commitment to promoting independence and enhancing the lives of individuals with complex needs.
•    Flexibility to work a variety of shifts, including evenings, weekends, and holidays.
•    NVQ Level 3 in Health and social care

To be considered for this position, you will demonstrate exposure to the following areas of expertise: mental health, complex health needs, physical health needs, hospital discharge to community rehabilitation, autism and learning disabilities, life-limiting conditions, working with children and young people / adults aged 18 – 65 and 65 and above.
